Why is this a REFILL - can your use it instead of non-refill

It is a refill bc the outside is not a hard case, however it can be used on its own. The lid that snaps shut is very durable and I have always just used them as is.

What's approximate expiration date if purchase today?

The one I just got, the wipes, has an expiration of 5/19.

Do you use this like toilet paper?

Not really directions says use up to 5 times daily or after hemorrhoid irritation after a bowel movement and when applying hemorrhoid ointment
