A serving is 2 capsules, so one serving is 1200mg.

Are the ingredients from the us and where are these manufactured ?

It says manufactured by Horbaach Manufacturing out of Las Vegas NV. The ingredients look to be plant based and is labeled vegetarian format but I have no idea country of origin.

Is this food grade or pharmaceutical (USP) grade ?

Don't really know the answer. I would think it's both food and pharmaceutical grade at least.
