How do you know what size to get?

You have to check the spacing on your guitar and compare it to this, I'm sure there is a size in the pictures.

There are several different sizes offered but no measurements or dimensions. How do I determine the correct model for my guitar? Thanks!

That I don't know. I bought a junker acoustic for $25 and just bought a set, they fit perfect. Bought a new bridge and saddle too...fit perfect. for a total of $45 got a great "new" guitar. I was gonna fill the holes w/ wood putty and drill new holes but, didn't need too.
