Q: Is this power distribution device spike protected? If so, is each outlet protected separately?

It does not provide surge protection

Q: Is it ok to connect the grounding cable for my two patch panels to the ground terminal on the pdu? Both are mounted in the same rack.

Always consult an electrician. However, I would run patch panels to the grounding bar or just ground your rack.

Q: can it be mounted so that the power cord faces out? i.e., can the ears be moved to the back instead of the front?

Yes. The ears are mounted using 2 Phillips head screws (each). Each ear has a total of 8 holes that allow for depth adjustment or reversal so that you can mount it either way. Thusly, you may remove, reverse, or adjust the mounting ears.
