could anyone tell me what the difference between 2019 and 2018

The Razer Kraken is really the Razer Kraken Pro V3. Razer went crazy with their names and confused everyone. So they went back to the original Razer Kracken name. This model has cool oval ear cups and an improved mic. I currently have a Razer Kraken 7.1 v1 and 7.1 v2. Gave the v1 to my wife and bought the new Razer Kraken 2019 for her to replace it. About 1 year ago I bought the Razer Kraken Tournament Edition with THX and returned it. The sound and the mic was horrible. The Razer Kracken 2019 uses this free software to simulate 7.1 surround https://www.razer.com/7.1-surround-sound . I should get my wife's new pair today.

Is the headset cable a combo jack or two separate headphone and microphone jacks?

Both. A combo 3.5 mm jack is attached to the Razer Kraken. An extension splitter cable is also included, which provides full audio and mic access for platforms requiring separate audio input and mic input ports.

Can I use these for ps4 and Xbox one s ??

Yes, Razer Kraken will plug into the 3.5mm headphone jack on the PS4 controller and Xbox One S controller.
