No it does not have a medicated smell. It actually has a very pleasant smell.

Does it contain protein?

In the list of ingredients it just lists the chemical name and doesn't have the word "protein" in it so very hard to tell if it's protein-related. But it does have collagen and that is a protein that is very good for the hair.

can you leave in over night?

I tried to do that,but it got a little messy. Try putting a small amount on your hair and brushing it through. This might be a great monthly treatment.
