No Paperwork comes with it at all it’s written right on the board voltages and such you need an amplifier to make it work

Does this take DC input

Yes, the white two pin connector is for 12v DC.

Does this preamp have balance control? Others, of similar configuration, have bass, mid, treble tone controls and volume.

it does not have a balance control. the ad is a bit misleading as they use balance in place of level. from left to right the controls are: hi-mid-low and volume. no assembly issues (no instructions included), sounds decent for the price, better with an IC upgrade.

Where is the Input for this preamp?

On the right hand side blue connector and the white connector to the left of the blue connector
