One at the end of the strap and a small square further in to stop it sliding down at an angle.A cut-out hole around the knee cap allows the material to stretch over the knee as the leg bends while also being reinforced to support the kneecap in place.The only problem was that the strap was a bit too tight for my husband's legs. The small Velcro square on the straps only just made it onto the rest of the body of the knee support.After thirty minutes of use though it was surprising that it was still comfy enough and hadn't restricted circulation.
