Q: Is there a filter with the device?

No-when you cough you have to remove the device from your mouth therefore mucous ends up in your mouth not the device.

Q: How do you clean it does it come with instructions

I have not tried to clean it, but it should be possible with an appropriately sized flexible brush. In my home, I am the only user, so I'm not concerned with spreading germs to others. Yes it comes with instructions. I did find it effective.
