Reasons are 1) On card reader u might have put the switch in lock mode. 2) if u suddenly removed ur memory card from mobile or if u removed your memory card from PC/Laptop without safe remove option. 3) memory card structure damage( if u drop memory card on floor or accidentally damage) Important points- 1) Never remove your memory card when the phone is "ON".
