It's as it says on the info - 1 sheet 12 X 15cm and no it's not made in the UK, made in China. You can cut the sheet to size - and it's also washable.

How often do you replace them please

You can take it off wash it with soap and water and reapply, the stickiness remains. I kept using the same piece for several weeks.
