One of my sleeves got damaged. and i only had it for 2 weeks. can i get a replacement?

I don't think that they were made to last forever, although two weeks doesn't seem like a very long time. Since they offer a "subscription service", it would seem that they do deteriorate after a while. I wash mine with a gentle soap and let them dry overnight and mine have lasted more than two months now. Keep in mind that a good orthotic insole can cost you ~ $60+...

i wear a 4 and a half, will these fit my foot?

Men's? Women's? If it's men's absolutely. If it's women's they should but maybe not as snug. I'm a 6 1/2 with fairly narrow feet and they fit perfect.

What is the thickness of support pad? Is it durable for running?

At it's thickest point, maybe about 5-7 mm. Probably durable for running, but I'm not a runner so I can't really say.
