This good for smoothing out a table top?

Short answer: yes. Longer: get a new iron. I bought the Hock iron and cap-iron set which turned this bad boy into a very good finishing plane. The included iron is just too soft. Even after diligently sharpening and lapping the iron, unless I was working on soft wood the edge would just curl right up.

Does the Lever Cap actually have a lever on it or does it have a screw? Some website reviews say it has a screw instead of a lever.

It has a screw for the lever. It was pictured as having a lever, but when I purchased it 2 years ago and opened it, I discovered it was not a lever type. Either way, it functions as it should be.

Does the frog have an adjustment screw or is it just 2 vertical mounting screws behind the plane iron?

It has two vertical mounting screws behind the iron.

Can this be used as a jack plane to flatten boards?

It is a jack plane. And that it is exactly what it is used for. Folloed by a smoothing plane (aprx 9" long)
