does it consume a lot of energy or as an usual device?

I use two of them in a large tank that holds an 8 inch amphibious turtle and never noticed any change in my electric bill.

does this work with sand on the bottom of the tank?

I have bare bottom tank but u suppose it would if your sand was settled and your raise the filter so it's not sitting I'm the sand. I have read over the forums that sand can clog just depends on your set up.

Do you have to buy specifically tetra fauna medium filter cartridges, or is there a different brand I can use?

My filter takes the large bags, not medium. I think any similar size filters would probably work but these filters are a very good value on Amazon--much cheaper than purchasing in my local pet store.
