Is it Possible to engrave pens with this machine?

Simple answer is NO, if you use the machine as a CNC router. You will be able to engrave pens with a laser. Saying that, you can add a laser attachment to the 4030. If you only want to engrave turned pens, you be better off getting a dedicated laser machine.

Does this need any form of extraction when cutting different materials or just a ventilated area?

Creates plenty of fine dust, the bits for the router they send are pretty crap, so if you upgrade make sure you get a router local to your country otherwise you will need a step down power unit as well. The extra cullet they send takes a DWP611 router which is America brand at 120v. You would be best served by some type of vacuum to keep the dust down. You could strap the hose to the router, or house in a unit that has an outlet for the vacuum.

what router does it come with and what power is it also what size is the collet that fits the router

It comes with a 300w spindle, they use er11 collets that you can get various sizes. It's suplied with a 1/8 collet if I remember correctly but not sure what size that is in metric.

Will it cut 18mm thick ply & mdf?

The bits they provided with the machine I received broke, so I upgraded the router and bits. To make the cut with what they supplied would require many small cuts to achieve the finale result. By the way if you replace the router be prepared for some serious sound proofing because it manes one hell of a noise.
