Does it come in 1 1/4 x 8 tpi threads

I believe the Pro-Tek version of the SuperNova2 only comes in insert-type--so get the insert you need for your application. I think that's true of the original SuperNova2 as well--but i don't know the full history.

Does this require an adapter? I have a Laguna 18/36 with a 1 1/4 spindle.

I had to buy an adapter to go from 1 1/8 to my spindle size of 1".

Will this fit a Jet 1221VS?

Yes, just make sure you get the 1" x 8 tpi spindle adapter. This is an "insert type" chuck, threaded to M-33, so you purchase the correct adapter depending on your spindle thread, the 1221 is a 1" x 8 tpi.
