How much assembly is required?

Assembly is pretty simple; although the instructions are a little ambiguous it will eventually be self-evident what you're supposed to do. The roof on the XL size is pretty heavy, so I needed a second set of hands to line everything up properly. Smaller sizes probably won't have that problem.

Is it supposed to arrive in a box with a picture of a tread mill? Not sure if I should open it.

How high off the ground does it sit? Would like to use it for feral cats/kittens in my neighborhood.

I agree with the 4". I use it for the outdoor cats - there is room for about 4 average size cats. I bought a cat bed to put in it for a softer stay. My cats use it and seem to like it. I keep mine under an awning so I didn't seal it. The rain can still get to it if it is storming hard but it does stay dry on the inside when that happens. The lid lifts on hinges. The cats lay as much on it as in it. Only one entry door, so no emergency escape if another critter tries to challenge the cat/dog. It wasn't too hard to put together and it's solid. I am happy with it.
