Why does one among those four pads has attached ring/bracket? why not it goes in pairs?

These metal bracket start rubbing against the brake rotors and make noise to let u know the the brake pads are worn out and need to be replaced

How long does this brake pads last ?

I put this on my Honda Accord exl 2014, it's all ready 17k miles works great, expecting to last at least 30k miles. Another vehicle also using Bosch brake pad and drive for around 26k miles. Very satisfied with this product.
