Does this surge protector just plug into my outlet or do I need to rewrite my outlet?

Remove the outlet cover typically held on by a slotted machine screw between the upper and lower outlets, then align the surge protector/outlet expander so that it plugs into the upper outlet with the lower peg into the secondary ground hole (if it has one, if not instructions say to cut the peg off) then use the combination slotted/Phillips machine screw provided to fasten it to the outlet using the same female threaded hole you removed the outlet cover screw from and tighten it.

Is this a smart device that works with alexa? can i turn it off/on with alexa?

No. This is an excellent non-smart plug expander, but does not have a network connection.

Can this be used on a cruise ship?

Yes if the particular ship upon which you travel has the standard American style plugs. I myself, have not seen any with the European conversions.
