Q: comes with all the accessories you see in the picture?

I would take that up with the seller, the kit includes all the accessories shown in the picture, you can buy the shield on it's own for under $2.00.

Q: Is it possible to just buy the shield by itself?

I don't think so, kind of a pain really, but it's actually less expensive for the manufacturer to sell accessories like this as a complete assembly. One less inventory item you know. In addition there will be very few folk who just want the shield alone.

Q: Can this shield be used on the Dremel flex attachment?

I do not have the flex attachment to answer this, but if the flex attachment has the same theads at the tool end like the Dremel, I would say yes. The shield attaches to the Dremel in the same way the flex tool does.
