I have shoulder length hair.  Can decide if the 2 1/8 or the 
2 3/4 inch would be better.

My hair is very long but it depends on the size of the wave/curl you want. It really doesn't matter the size of the brush because you can change the width section of hair you use to wrap around the brush when you are blow drying it. You can opt for a one inch section of hair or more. The bigger the barrel of the brush, the more hair you can wrap around it. If it's a smaller barrel you can't wrap a big section of hair. For me personally if you are in doubt go smaller. I actually have multiple sizes of round barrel brushes if I want to make tight waves or not. If you are mainly just folding your hair under and then straightening with a flat iron, opt for a bigger barrel.

How do you use it?  I thought there was a button or something on it to start hot air blowing...
No instructions....Do I need a blow dryer with this?

It is a regular brush no buttons, yes you use a blow dryer to dry your hair but it really does leave the hair very soft .
