How do I pan the microphone output signal from USB to the middle? It's all to one side as a stereo channel.

That's something you need to with settings in your DAW.

Would this work with an ipad or an iphone? Will the iphone or ipad recognize this in let's say Garageband software?

I've used it on an Apple laptop in GarageBand, but I really do not think it was meant for use on cell phones. At the very least, it does not have the connectors for either.

I lost the usb, can you tell me where I can get a replacement or what cord to look for

USB A (male) to USB B (male). It’s the same as a standard printer cable. You can get them at any major store with an electronics department, or on line at Amazon or Monoprice.

Does it require software for pc? Does it come with it or where can I download it at?

It comes with drivers and a trial version of ProTools, but I have a Mac so I didn't use any of it.
