I have an APBT and it fit her neck. It is adjustable.

I have an Akita puppy (6 mos 78lbs) he's a puller, with strong Akita traits (plush thick, fur extra scruff at neck) wondering if this will work?

I didn't have much luck with my husky mix (thick fur,) but this works on my pit mix (thin fur.) I got a metal prong collar after the pit mix broke my husband's arm (pulled him down after she lunged at a passing dog,) and trained her on that, but this plastic one is now pretty effective on her. I'd probably go with a metal prong for an Akita.

I have a medium sized dog. Is it possible to buy a large collar and then take some links out to adjust for size?

Yes. I had to add links to my dogs collar as he grew. It is very easy and that was one of the things I really liked about this collar.
