Can this be applied to scalp?

I think you'd find it would be quite greasy - it does contain paraffin, glycerin, and other fairly greasy substances. If you suffer from a dry scalp, I would suggest plain olive oil - just a drop, rubbed between your fingers and then massaged into your scalp. Hope that helps.

Does this product come as shown in the second picture as a jar? or with pump dispenser as in the first picture?

Back when I bought this, the picture only included the jar. And what I got was only the jar. You'll have to confirm with seller.

Is this Paraben Free?

QV cream contains less than 0.5% parabens. This is the best lotion I've ever used, have to admit.
